We welcome the opportunity to serve you and your patients, and are committed to providing attentive, professional, and personalized care.
We work in close partnership with each of our referring physicians and always retain open communication to ensure patients receive the best possible treatment.

Information for Patients
As we are a specialist medical practice, we do require a referral from either an optometrist, general practitioner, or another specialist.
If you have regular appointments with us, then a new referral is required every six-months.
*Please note our office requires 24 hours notice for all cancellations. Appointments are in high demand, and your early cancellation will give another person the possibility to have access to timely medical care. Late cancellations will be considered as a “no show” and charged $50.00
